An employee was travelling in a stand-up forklift when he came into contact with another parked forklift that no one was operating. His foot was outside the operator compartment at the time and was crushed, requiring surgical amputation.

An employee was standing on top of water pumps to reach a power disconnect. The frame bolts on one of the water pumps were corroded, and the employee's weight caused the pump to break loose and release hot water. The hot water contacted the employee, causing second degree burns to the legs and the lower back.
An employee was riding on a powered pallet jack near a sewer drain. The pallet jack hit a dip near the sewer drain, and the employee s left foot hit a clamp truck parked in a charging station. He suffered a cut to the big toe, as well as fractures in the toe and the foot.
