The belt started, causing the employee to lose balance and fall approximately 2 feet. The employee was hospitalized with a fractured ankle and required surgery.

The employee lifted the auger to position it when the auger feed initiated and energized the auger. His hands were pulled into the auger, resulting in hospitalization with a partial amputation of the right thumb.
