Final narrative
A maintenance employee was cutting a molded plastic platform using a handheld circular metal saw when he lost control and dropped the saw. The saw lacerated his right leg below the knee. He was hospitalized.
An employee was connecting a hose to a production tank to transfer sulfuric acid when the acid released and contacted the employee, causing chemical burns to the lower extremities.
At approximately 12:45 p.m., on 23 July 2015, an employee was gathering nitric acid at 67% into an 8oz bottle. The employee filled the bottle and it splashed acid in
the employee's face, causing chemical burns.
The injured employee was working the 2nd shift (2:30pm - 11:00pm) packaging bags of lime (material). The employee's arms came in contact with the lime. He was later hospitalized.
Two employees were lifting a roll of steel poly bar (50 inches and approximately 150 pounds) from ground level to a height of 4 feet at the 66-inch rubber calendar.
