An employee was moving a portable heater when his finger was caught in the grating protecting the fan blade on the rear of the heater. His left ring fingertip was amputated with no loss of bone.

An employee was visiting various construction sites. After parking their vehicle, the employee walked across the road to a construction site and was struck by a car, suffering a broken left femur.

An employee was loading a side dump truck while working on a median at the intersection of Lincoln and Lamay in Ft Collins. The driver was given the signal to pull forward just as the injured employee slipped on the curb. The dump truck ran over the employee's leg.

An employee was in the packaging area in a warehouse transporting a 5-foot tilt and roll ladder back to its storage location. The employee was pushing the ladder while standing inside of it when it began to tilt over. As the employee reached to stop the ladder from tilting, his right hand contacted the metal serrated platform. The ladder continued to tilt and as he pulled his hand free, the employee's right middle fingertip was amputated.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was installing insulation in a wall when she tripped on a hole (12" deep) and fell, hitting her head on the concrete floor. The employee sustained fractures to her leg and wrist, and lacerations to her head. The employee was hospitalized.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.

An employee was operating a power drill when it slipped. The drill bit punctured the employee's hand and they were hospitalized.

The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
