The rod became jammed and struck the right side of the employee's head and face. He fell to the ground and lost consciousness. The employee was hospitalized with fractures to the jaw and orbital bone, and multiple lacerations.

Two employees were pushing a lawn mower into a utility truck. The injured employee placed their hand at the left side of the lawn mower deck when they were struck by a blade. The employee's left middle and ring finger were lacerated.

The employee was pushing the ladder while standing inside of it when it began to tilt over. As the employee reached to stop the ladder from tilting, his right hand contacted the metal serrated platform. The ladder continued to tilt and as he pulled his hand free, the employee's right middle fingertip was amputated.

An employee was using a push mower to cut the grass. The employee picked the mower up to move it and sustained a right middle fingertip amputation.

An employee was performing preventative maintenance on a deflation fan, belt, and pulley. While working on the fan, the rotor in the fan unexpectedly began to spin due to the activation of a damper within the baghouse. The spinning of the fan caused a belt pulley to begin spinning, which made contact with the employee's hand, resulting in an amputation to the right middle fingertip.
