While disconnecting a service line, an employee was struck in the neck by an energized line (8000V) due to a fallen pole and they were electrocuted.

An employee was completing various work tasks outside throughout their shift and experienced heat-related symptoms.

Two employees were responding to a single vehicle accident involving Entergy facilities. The vehicle's fender was entangled with a guy wire supporting a primary conductor. The two employees were working to release the tension and temporarily move the guy wire. When the tension was released, the guy wire contacted the primary conductor and an employee sustained electrical injury. The injured employee sustained a mild burn to the palmar aspect of the left hand and the left foot where voltage was discharged. The employee was also noted to be in atrial fibrillation.

An employee was attempting to disconnect electrical service at a residence when a dog attacked the employee. The employee suffered puncture wounds on the left forearm, a crushed right index fingertip (bone), and tissue and nail bed damage (pulled off bone but not completely torn).

An employee was laying down bridging (angle iron that is welded onto open web joists). He was moving backward toward the edge of a joist when his foot slipped. He lost balance and fell backward away from the structure and landed on the ground about 30 feet below. The employee sustained a broken right leg. The employee was wearing a harness but it was not connected at the time of the fall.

An employee fell approximately 30 feet from a roof, resulting in head and spine injuries.

On June 16, 2023, an employee was checking the I-beams on a bridge. He unhooked his harness to move across the section of the second beam. While hooking his harness on the third beam, his foot hit the cable that was connected to the beam and fell approximately 25 to 30 feet to the muddy clay floor below. The employee sustained a left hip and femur fracture.

An employee was using a 60-foot ladder to access the fourth floor of a building in order to paint the balcony. The employee was holding onto the handrail while painting it when the handrail brackets broke, causing him to fall approximately 30 feet to the ground and land on top of air conditioning units. The employee suffered severe lacerations on the face and fractures to the cheekbones and eye bones. The employee was hospitalized.

An employee entered a portable structure that was being rigged up to a crane. The crane began lifting the portable structure with the employee inside when the structure fell 15-30 feet to the ground. The employee sustained a left leg fracture, a dislocated right ankle, and back injury.

An employee contracted by another company was conducting work on the platform of the plane train when he fell off the platform, resulting in a fracture and contusion to his face.

Two employees were setting up an inspection camera over a manhole. One of them tripped and fell into the manhole, landing about 20 feet below and suffering a broken left ankle, a sprained right ankle, and a laceration to the back of the skull. He was hospitalized.

On 11/9/2023, an employee on an extension ladder was reconnecting power to a residential meter pole. The wooden pole shifted and caused the employee to fall from the ladder to the ground. The employee sustained a fractured right tibia.

An employee was on a ladder re-routing electric service meter equipment. As they were removing the old service from the meter, the employee slipped from the ladder and landed approximately 5 feet below on concrete. The employee sustained broken bones in the lower left leg.

An employee was grinding a gusset plate with a disc grinder when the gusset got caught, pulling the employee's finger into the machine. The employee sustained an amputation to the right index finger.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A security employee was holstering her firearm when the gun discharged a bullet into her right leg. The employee was hospitalized.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
