An employee was attempting to clear a jam of material from a trim conveyor on the back side of a strip caster when his finger was caught in the chain drive sprocket. His left ring finger was amputated just below the first knuckle. The chain was unguarded and the conveyor was not locked out at the time of the incident.

On July 23, 2025, an employee was conducting HVAC work in a mechanical room that had an ambient temperature of 110- to 120-degrees. At the end of the day, he experienced dizziness, cramping, and became ill. The employee was hospitalized for heat stress and dehydration.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

An employee was demonstrating how to disassemble and reassemble a metal filter unit from the cathode powder transfer line. During the open/close operation of an automated valve, the employee s latex glove became caught. The employee sustained a partial amputation to the left index finger.
