On January 2, 2018, an employee was operating a crane to move a scrap bin to a dock when the scrap bin struck some steel and began to rotate. As the employee attempted to stop the bin from rotating, the employee's pinky finger became caught between the scrap bin and a table machine, resulting in an amputation.

Kloeckner Metals Corporation

An employee was placing 4x4 timbers under a steel plate that was being loaded onto a trailer for delivery. The plate was raised about 12 inches by four chains with hooks on the four corners of the plate. One side of the plate was flat and the other side was raised; the middle section was beveled upward. While the employee was placing the timber under the plate with his left hand, the plate lost tension and fell, crushing the tip of their left index finger. The employee's fingertip was amputated.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.
