An employee had been working on a vehicle. He was unlocking the mechanism of the vehicle lift when the lift engaged; the hammer came down and struck the end of his right middle finger. The fingertip was amputated at the base of the nail.
An employee was putting tires onto storage racks. The employee slipped and grabbed a rack, trying to prevent a fall. His left pinky finger was caught by the rack and amputated to the first knuckle when he fell approximately six feet.
