On July 19, 2025, an employee was filling a wheelbarrow tire with air when the tire exploded, resulting in severe injuries to their right hand including a partial amputation.

An employee was unlocking a yard gate from within a car. His finger was caught between the gate and the car, and the fingertip was amputated.

They planned to ignite the brush pile by creating a fire-line or trail with an accelerant so that the pile could be lit from a distance. After applying a gasoline-diesel mixture to the pile, one employee used a cigarette lighter to ignite the pile. Two employees suffered multiple first-degree burns to the face, ears, hands, chest, and abdomen.

An employee was transporting used filter media (containing pyrophoric iron sulfide) from an inlet gas separator in the bed of a truck. The material ignited. While the employee was removing items from the truck bed, a gas can was exposed to the fire and off-gassing vapors ignited. The employee suffered first- and second-degree burns to both arms, the chest, the neck, and the right ear. The employee was hospitalized.

An employee was using a cutting torch to heat up bolts that had seized up. The bolts caught on fire in a flash burn, and the employee suffered burns to the arms and elbows.

On August 2, 2025, at 4:59 PM, a contract painter was painting an aircraft using an electrostatic paint gun in the paint booth hangar. He was placing the gun into a container of methyl ethyl ketone to clean when the solvent ignited. The employee was hospitalized with third-degree burns to his arms and legs.

An employee was outside, participating in fire extinguisher training onsite. She extinguished a fire and then the chemical from the fire got in her face. She started coughing and was hospitalized with trouble breathing.

An employee was repairing the suspension of a semi-trailer. He was installing an air bag to the suspension when his left ring finger was crushed between the air bag and the suspension mounting beam. The employee was hospitalized.

An employee was refueling a pressure washer when it ignited and exploded. He suffered multiple third-degree burns to his chest and hands.

A mechanic had just finished a repair on a straight truck with a flatbed , which had been lifted using a bottle jack. As the employee was about to remove the jack, it failed. The truck shifted forward, dropped down, and struck the employee's chest. He suffered broken ribs and a broken shoulder.

An employee was performing diagnostic work on a vehicle engine. As the employee was leaning over the front of the vehicle, the engine was activated. A fuel leak had produced fumes around the engine, and the ignition spark ignited them. The employee suffered second-degree burns to both hands, both biceps, and the upper chest area.

An employee was working on a control line when it got jammed with steel strips. They went to unjam the machine and suffered a fingertip amputation to the left middle finger.
