An employee was walking backwards while operating an overhead crane when the employee tripped and fell over a pallet. The employee's right elbow struck the frame of the crane and suffered a fracture.
An employee was operating a tape cutting machine, which automatically cuts 2-inch cloth material into predetermined lengths. The material became jammed in the blade, which froze in the partway down position. The employee began working to unjam the material. The machine activated, causing the blade to become free. The blade came down and amputated the employee's left index fingertip above the first knuckle and the tip of their left thumb.
