The material became jammed in the blade, which froze in the partway down position. The employee began working to unjam the material. The machine activated, causing the blade to become free. The blade came down and amputated the employee's left index fingertip above the first knuckle and the tip of their left thumb.

An employee was working in a test bay disassembling an iron spool when the spool shifted and his left little finger got caught between flanges, resulting in a degloved finger that had to be partially amputated.
