The material became jammed in the blade, which froze in the partway down position. The employee began working to unjam the material. The machine activated, causing the blade to become free. The blade came down and amputated the employee's left index fingertip above the first knuckle and the tip of their left thumb.

An employee was operating the laminator and removing a stuck part from within the machine. The bottom plate of the machine raised and crushed the employee's left hand.
