An employee was pushing a cart when he hurt his hand, became lightheaded, and fell, striking his head on the floor. He fractured his skull and was hospitalized.
An employee was filling a tank with nitric acid. The nitric acid sprayed out of the ball valve onto the employee's back and leg. The employee received second degree burns to the buttock area, requiring hospitalization.
