An employee was cutting sanitary pipe when he began to feel overheated and experience a headache and difficulty breathing. He then sat in the air conditioned work truck to cool down until he returned to work. The employee was carrying tools when symptoms reoccurred and he began to experience muscle cramping, resulting in hospitalization for heat illness.

Employees were installing an intermediate driveshaft in the driveline of a dump truck. One employee went under the truck to visually observe the yoke and their left thumb was crushed/lacerated in a pinch point between the universal joint cap and the yoke.

An employee was utilizing a battery-operated circular saw to cut a 6-inch back-form board when the saw blade struck their left hand, resulting in lacerations to the middle and ring fingers.

A crew was rigging an RCP pipe to be lifted by a backhoe. The injured employee signaled for the operator to lower the pipe. He was adjusting the choker cable on the pipe when the pipe was lifted again. His left index and middle fingers were then crushed between the cable and the pipe, resulting in fingertip amputations.

An employee was preparing to connect storm pipe to a structure. The employee went to connect the choker cable to the excavator bucket and the pipe slipped and struck them. The employee sustained a crush injury to the left foot that involved multiple fractures.

On November 16, 2023, at around 3:30 p.m., a subcontracted employee was painting a door jam on a three-story house when he fell 20 feet through an opened elevator shaft to the floor below. The employee sustained fractured vertebrae that required hospitalization.

Employees were removing and replacing skylights on a metal roof. The injured employee's lanyard became tangled and they fell through the skylight to the ground 16-18 feet below. The employee suffered fractures to the wrist, ribs, and hip.

An employee was requested by a client to string wire on top of an auditorium. He used a boom lift to access a beam and then exited the boom lift to walk on the beam. He then stepped down on a drop ceiling, lost balance, and fell. The drop ceiling caved in and the employee fell 20 feet onto bleachers sustaining injury to his abdominal area and lower left leg. The employee was wearing a harness, but it was not connected to an anchoring point at the time of the fall.

An employee was installing decking (steel grates) in a new construction steel framed building. The employees were working approximately 22' above the next lower level, which was a concrete ground floor. The decking was pre-cut with the floor opening prior to installation. Shortly after installing the floor opening, the employee fell through the floor opening, approximately 18' onto a section of metal ductwork, resulting in fractures to the skull, ribs, thorax, and lumbar vertebra, as well as an ear laceration.

An employee was taking measurements of an area for drywall when they stepped backward into a partially covered hole in the floor. The employee fell approximately 17 feet to the level below, sustaining a forehead fracture and avulsion, a right shoulder dislocation, a left forearm fracture, left knee tendon and meniscus tears, as well as multiple contusions and abrasions.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.

An employee was assisting the crew with patching and restoring a roadway after the installation of a sewer line. He was helping place and level the blacktop/backfill material over the trench area to bring the road surface back to proper grade. The employee was walking between a forward-moving dump truck and a dirt pile when his right foot was run over by the truck s wheel, causing him to fall. The employee sustained injury to his foot.

An employee was rigging I-beams. An I-beam slid off the stack of I-beams and fractured the employee's leg.

An employee was operating a skid steer with a bucket attachment at a demolition site. While driving it to the rear of the property to park it, the employee came into contact with a live electrical transformer unit that was located outside of the facility. The employee sustained electrical burns on his hands extending up to his elbows and a fractured ankle.

An employee was installing a door on a semi-trailer. They were using a forklift to lift the door to the height of the trailer when the door became jammed between the forklift mast and the hinge on the trailer. When the employee went to dislodge the door from the mast, the door fell and crushed their foot. Their big toe was amputated. The employee was hospitalized and their big toe was surgically reattached. The employee's foot and other toes also required surgery.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
