An employee was walking over to check for leaks on a gauge. The employee noticed three lines/hoses had collapsed. The blender was on and when the valve opened 100 pounds of pressure hit the hoses causing them to jump up and strike the employee breaking the right tibia and fibula.

An employee was walking toward a pump on which he was installing a new suction manifold. He tripped over the old manifold, lying on the ground, and fell. He suffered a compound fracture to the left forearm and was hospitalized.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.
