An employee was trying to grab a box overhead while on a step ladder when he lost his footing and the step ladder tipped over. He fell to the concrete floor, lacerating both elbows and breaking his left elbow. He was hospitalized and had surgery.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
