An employee was setting up a line and noticed that a polytube above a dust hopper feeder had pulled into the transition piece. He removed the lid from the feeder and was reaching in to pull the polytube out when the valve closed on the ends of his index and middle fingers, amputating the pads of his fingers.

On 02/07/19, at approximately 11:45 AM, an employee lost control of a forklift. She attempted to brace herself with her left hand, which struck a desk and was lacerated.

On March 23, 2016, at approximately 1:30 a.m., an employee was cleaning out a vertical blender. When she opened a 10-inch x 10-inch door on the side of the blender, her right hand was caught between the side of the blender and the moving auger, amputating four fingers. The machine was energized at the time of the incident.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was operating a drill and tap machine. He was placing a part with his right hand and removing a part coming in on the left side. He saw a part that was crooked and went to straighten it. The machine clamped down on his left index finger, degloving the top part of the finger and resulting in a partial amputation.

An employee was performing maintenance on a roll cutting machine in a warehouse. The employee was removing excess grease from the bearings when the machine's chain caught their right index finger. The fingertip was amputated by the chain and sprocket.

During fiberglass work, an employee mixed a methyl ethyl ketone peroxide (MEKP) catalyst with a cobalt chemical that they thought was a dye. A chemical reaction occurred, resulting in an explosion. The employee sustained second-degree burns on his back and a blister on his right arm.

An employee was walking when she tripped over a box on the ground and fell. The employee was hospitalized with a severe dislocation to the hip and leg that required surgery.

The employee underwent a medical amputation from the top knuckle to the tip of the finger.

An employee was climbing down a ladder when it slipped. The employee fell, suffering a broken scapula and broken ribs. The employee was hospitalized.

An employee was climbing down a 15-foot multipurpose ladder. The employee fell about 8 feet, landing on the concrete floor and the ladder itself. The employee suffered several injuries, all on the right side: broken ribs, a broken sternum, a broken temporal bone, broken shoulder, multiple fractures to the cheekbone, and a brain bleed.

An employee was rolling up the landing gear on his trailer when the handle began to unwind and struck his face, fracturing his jaw. The employee was hospitalized and required surgery.
