An employee was replacing the shell body of a ride vehicle. He was maneuvering in-between the vehicle and the body to hook a sling to the body prior to lifting it onto the vehicle. He had one hand on the shell body and was stepping over the corner of the vehicle chassis. As he reached for the farthest hook-up point, the shell body shifted and he fell back. His lower left leg struck the shell body positioning pin, resulting in a laceration.

On March 15, 2021, an employee was helping demolish a concrete floor when a 650-pound, 3-foot by 3-foot concrete slab slipped off a forklift and landed on his right foot. The foot was broken and he was hospitalized, requiring surgery.

An employee was using a mechanical meat slicer to slice sausage, placing the sliced sausage into a container. The container slipped and the rotating blade cut his right thumb, amputating the tip.

An employee was cutting paper with a paper cutter when the cutter slipped and amputated the employee's left thumb tip.

A service employee was walking close to the edge of a dock when he stepped off the edge but did not fall off the dock. He fractured his right ankle in two places and required surgery.

On 12/22/2023, an employee was carrying bins filled with pasta to a refrigerator. As he lifted a 20-pound bin, he felt pain in his lower back. The employee sustained a herniated disc.

An employee was lifting a patient when they pulled a back muscle. She suffered back pain and spasms radiating into her leg with weakness and numbness. The employee's L3-L4 disc was herniated and she was hospitalized.

An employee was lifting boxes when they suffered a strained back, resulting in hospitalization.

An employee was lifting a bundle of insulation with a colleague when he felt severe pain in the right side of his lower back/hip region. The employee sustained a severe muscle injury.

On October 11, 2025, an employee was walking through a parking lot when they tripped on a pavement joint and fell, resulting in a fractured right ankle and a left knee injury.

An employee was unloading merchandise from a pallet when she tripped over the pallet and fell forward. Her head and face struck the floor of the stockroom, resulting in multiple head and face injuries that required hospitalization.

An employee tripped on an incline/ramp at the door to a building, fell, and suffered a broken left arm.

At the end of a shift on the downhill coaster, an employee was waiting for an empty cart to arrive at the bottom so he could ride it back up the hill. The cart arrived with its seat belt safety lock engaged. The employee boarded the top of the cart and began working to disengage the lock by reaching under the cart. His right hand and two fingers got caught between the rear wheel of the cart and the coaster rail. His ring and little fingers were crushed and lacerated, resulting in surgery and partial amputation of the ring finger.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.
