An employee was undergoing SWAT paramedic training. He was on the monkey bars when he swung his feet up, slipped and fell about 7 feet off of the monkey bars, and landed on his head. He fractured his D6 and D7 vertebrae, requiring surgery.

Petroleum Service Corporation

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee exited the ambulance van she had been driving and went to lower the hydraulic wheelchair ramp/lift on the side of the vehicle. The lift would not lower into position. She went around to enter the van to continue troubleshooting the lift when the van began to roll backward as the employee was partially out of the driver s side door. She reached in to stop the vehicle but the transmission shifted into reverse. The van turned and backed across a residential street, dragging the employee approximately 15 feet until it went up on a curb and the employee s left leg was pulled under the left front tire. She was hospitalized with fractures to her right wrist, right hand, and left fibula.
