An employee climbed onto a catwalk in the kill room to get a bird out of the facility. The employee stepped on some foam in the overhead thinking it was a solid floor and fell approximately 18 feet to the floor, suffering a fractured elbow and head contusion.

On November 16, 2023, at around 3:30 p.m., a subcontracted employee was painting a door jam on a three-story house when he fell 20 feet through an opened elevator shaft to the floor below. The employee sustained fractured vertebrae that required hospitalization.

Employees were removing and replacing skylights on a metal roof. The injured employee's lanyard became tangled and they fell through the skylight to the ground 16-18 feet below. The employee suffered fractures to the wrist, ribs, and hip.

Perfectly Safe Systems

An employee was requested by a client to string wire on top of an auditorium. He used a boom lift to access a beam and then exited the boom lift to walk on the beam. He then stepped down on a drop ceiling, lost balance, and fell. The drop ceiling caved in and the employee fell 20 feet onto bleachers sustaining injury to his abdominal area and lower left leg. The employee was wearing a harness, but it was not connected to an anchoring point at the time of the fall.

An employee was installing decking (steel grates) in a new construction steel framed building. The employees were working approximately 22' above the next lower level, which was a concrete ground floor. The decking was pre-cut with the floor opening prior to installation. Shortly after installing the floor opening, the employee fell through the floor opening, approximately 18' onto a section of metal ductwork, resulting in fractures to the skull, ribs, thorax, and lumbar vertebra, as well as an ear laceration.

An employee was taking measurements of an area for drywall when they stepped backward into a partially covered hole in the floor. The employee fell approximately 17 feet to the level below, sustaining a forehead fracture and avulsion, a right shoulder dislocation, a left forearm fracture, left knee tendon and meniscus tears, as well as multiple contusions and abrasions.

An employee was grinding meat in a meat grinder when the auger caught his nitrile glove and pulled his arm into the machine. The employee sustained amputation of their left hand and arm to halfway up the forearm and required surgery. The machine was not guarded at the time of the incident.

An employee was looking at racks in a shipping area. He stepped back and his foot got caught in a pallet, resulting in a fractured right heel.

An employee was cutting a sow ear tag when the tag remover tool slipped and cut his left index finger, resulting in a partial fingertip amputation.

An employee was cleaning a sheeter transfer conveyor. He was cleaning the conveyor support bars when his left hand was caught and pulled into a conveyor sprocket. He suffered a fracture, a laceration, and tendon/nerve damage to the hand and was hospitalized.

A warehouse employee was operating a forklift in the freezer area. While making a turn, the forklift slid on condensation that had accumulated on the freezer floor. The forklift struck a yellow pole that designates the exit path and the employee s left heel became caught between the forklift and the yellow pole, resulting in a heel pad avulsion, or rupture of the heel fat pad. The employee's injury required surgery and hospitalization.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
