An employee was going between a ladder and a wall to repair a wall in the utility room. A brad-nailing gun was hanging on a piece of wood on the side of the ladder. The employee contacted the brad nailer and the trigger was pushed into the wooden piece. The brad-nailing gun actuated and shot a 2-inch brad into the employee's abdomen.

An employee was moving a step ladder to a different location. A brad nail gun with an air hose attached was hanging on the side of the ladder. When the employee climbed the ladder, his right leg hit the safety tip of the nail gun and the nail gun shot a brad nail into the employee's right leg.

An employee was building walls for mobile home manufacturing using a 2 1/2 inch staple gun when the gun activated and a staple lodged into their right kneecap.

A forklift struck a bar under a bay door. The bar fell and struck an employee's head, breaking the employee's skull. The employee was hospitalized.

On May 27, 2022, at 2:30 p.m., an employee was descending a ladder while holding a nail gun when the nail gun discharged and a 2-inch nail entered the employee's upper-right abdomen, resulting in hospitalization.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

On September 5, 2025, an employee was installing gang plates on a structure when the side walls fell and hit her head. She then fell onto her back and the side wall pressed her to the ground, resulting in hospitalization due to an ilium fracture and a transverse process fracture on the right side.

An employee was blowing cellulose insulation into the open attic area of a manufactured home. While the employee was holding the insulation hose, it caught on the guardrail of the catwalk/suspended walkway leading into the attic. The tugging motion caused the employee to lose their footing and step onto the sheetrock ceiling. The employee fell through the ceiling to the ground about 8 feet below and suffered a broken lower left leg. The employee was hospitalized.
