An employee was helping to assemble telescoping boom sections. Another employee was using a forklift to push the sections of the boom together. During this operation, the injured employee's right hand was inside the boom section to lift up and dislodge the chain, which had become caught on the outer boom. The employee's right middle finger was then caught between the gear and the chain, resulting in an amputation.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was working in the food service warehouse when he experienced chest pain, difficulty breathing, and lost consciousness. The employee sustained carbon monoxide poisoning.
