An employee was cutting through two secondary wires with a battery-powered cutting tool. The wires were energized at 120/240 volts; when the employee cut through the insulation around a wire, an arc flash burned the employee's left thumb.

Two employees were pulling and connecting electrical wires. They came into contact with a live, parallel wire and both suffered electrical burns. They were hospitalized.

An employee was on the roof of a house, securing overhead electrical service to a mast. The mast failed and broke off from the attachment point, causing the employee to fall from the roof. The employee suffered a broken left femur and was hospitalized.

An employee was in the packaging area in a warehouse transporting a 5-foot tilt and roll ladder back to its storage location. The employee was pushing the ladder while standing inside of it when it began to tilt over. As the employee reached to stop the ladder from tilting, his right hand contacted the metal serrated platform. The ladder continued to tilt and as he pulled his hand free, the employee's right middle fingertip was amputated.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.

An employee (a lineman) was working to restore power by installing a hand line when he contacted a high-voltage device (7,200 volts), resulting in electrical burns to his upper body and hands.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
