An employee was using a manual pallet jack to load the delivery truck. The employee moved backwards inside the cargo area while pulling the pallet of merchandise with the manual pallet jack, stepped off the back edge of the box truck bed and fell approximately four feet to the ground. The employee sustained a compound fracture to the left ankle.

An employee was carrying a box of blinds (approximately 36 pounds and 6 feet long) down a small flight of exterior concrete stairs at the customer s preferred delivery location. He fell down the last two steps to the concrete ground below. The employee sustained a dislocation of his right knee and a torn patellar tendon, requiring hospitalization and surgery.

An employee was operating an electric pallet jack (EPJ) and making a U-turn when they became caught between the pallet jack and racking. The front part of their foot was pinned between the racking corner guard and the EPJ platform. The employee sustained lacerated tendons to the right big toe that required surgery.

On September 18, 2023, an employee was moving pallets of sinks with a double electric pallet jack when their left foot was pinned between the pallet jack and another pallet. The employee's left ankle was broken.

An employee was offloading materials to deliver to a customer when they fell from the back of a box truck to the pavement. The employee sustained a clavicle fracture, fractures to ribs 2-9, and a head laceration.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

Two employees were working on a forklift with its mast in the air when a chain came loose and the carriage fell down on them. The injured employee was hospitalized with a fractured left ankle, a laceration on the left side of their forehead, and a fractured L1 vertebra.

An employee was operating a machine used to roll erosion control fabric. Her left hand was caught and crushed in the rollers. She was hospitalized.

An employee was being trained on operating a powered industrial truck (PIT) with a side-saddle stance. The employee was instructed to move the PIT s joystick back and forward to get a feel for it. The PIT went backward through a dock door that was approximately 3 feet off the ground. The PIT ended up on the employee. The employee was hospitalized with an amputation below the left knee, third-degree burns to lower extremities, and multiple fractures to the lower extremities.
