An employee was operating a forklift and reached to adjust the materials on the forklift. Their knee hit the lever and the materials on the forklift fell onto their right hand. The middle and ring fingers were crushed.

An employee was setting up placers on the ground for bundles of sheetrock (gypsum) to be laid. The employee placed plastic on a piece of gypsum that was not covered. A forklift placed the load down and the employee was caught by the bundle of gypsum. The employee's left little finger was amputated.
