An employee was standing in the doorway of a guestroom they were cleaning. Their left hand was on the door frame while they used their right hand to reach for a cleaning bottle. The employee's left little finger was caught between the door and the door frame and they suffered a soft tissue avulsion of the finger requiring surgery.

An employee was opening a cabinet to retrieve work items when a lamp that was on top of the cabinet fell and struck the employee's head. The employee sustained a bump on the head and lost consciousness. The employee was hospitalized for a head injury.

An employee was closing a ferry pilot house door. The door caught the employee's right little finger, causing the amputation of the fingertip.

An employee slipped/stepped off the edge of a sidewalk, lost their balance, and fell forward onto the sidewalk. The employee struck their face and head on the sidewalk. The employee sustained a brain bleed, lip laceration, and pain in shoulder and was hospitalized.

An employee tripped and fell while walking through an offstage corridor, striking their head on the ground and suffering a concussion.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was in a paint booth to change filters underneath the booth. She took a step when her right foot fell into an opening in the floor, resulting in a right ankle injury that required hospitalization and surgery.

On October 11, 2025, an employee was walking through a parking lot when they tripped on a pavement joint and fell, resulting in a fractured right ankle and a left knee injury.

An employee was unloading merchandise from a pallet when she tripped over the pallet and fell forward. Her head and face struck the floor of the stockroom, resulting in multiple head and face injuries that required hospitalization.

An employee tripped on an incline/ramp at the door to a building, fell, and suffered a broken left arm.

At the end of a shift on the downhill coaster, an employee was waiting for an empty cart to arrive at the bottom so he could ride it back up the hill. The cart arrived with its seat belt safety lock engaged. The employee boarded the top of the cart and began working to disengage the lock by reaching under the cart. His right hand and two fingers got caught between the rear wheel of the cart and the coaster rail. His ring and little fingers were crushed and lacerated, resulting in surgery and partial amputation of the ring finger.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
