Final narrative
An employee was picking up a box that had fallen off a pallet. A nearby stacked pallet of boxes became unstable and fell, breaking the employee's femur.
Employees were conducting inventory in the rail room. A forklift operator had moved four pallets of boxes into a storage location (bulk stacking) and pulled away. The injured employee entered this location to perform cycle counting/inventory and the stack of pallets fell forward, landing on the employee's right leg and back. The employee was hospitalized for a compound fracture to his right leg that required surgery.
