Employees were conducting inventory in the rail room. A forklift operator had moved four pallets of boxes into a storage location (bulk stacking) and pulled away. The injured employee entered this location to perform cycle counting/inventory and the stack of pallets fell forward, landing on the employee's right leg and back. The employee was hospitalized for a compound fracture to his right leg that required surgery.

An employee was operating a forklift in the loading dock area. They put a loaded pallet of product into a pallet invertor, then backed out of the pallet, and exited the forklift. Another forklift then struck the employee's left leg and pinned it against the employee's forklift. The employee's leg was lacerated.
