An employee was bitten by dog.

On Nov. 23, 2014, at approximately 9:00 PM, an employee sustained an animal bite (cat) to her right finger. She was treated and released at the ER. Later, due to swelling, she was admitted to the hospital and underwent surgery on the tendon in her right finger.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.

An employee was unloading materials from a flatbed trailer when they fell from the trailer and landed on the ground. The employee sustained a fractured left leg.

An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.

An employee was coming down a portable ladder when they slipped and fell to the ground, resulting in a right forearm fracture.

An employee was restraining a cat that was thrashing in its cage while recovering from anesthesia so the veterinarian could administer sedation. The cat bit the employee on their left thumb, resulting in hospitalization due to an infection.

On September 6, 2025, an employee was administering medication to a cat when they suffered a bite to their thumb, resulting in hospitalization.

A client arrived for an appointment and was getting out of their car when their cat carrier broke and their cat went under the car and into the engine area. The injured employee was able to scruff the cat and started to pull it out. The cat scratched and bit the employee's hands. One wound was close to a knuckle and immediately started to swell. The employee was hospitalized for treatment.

An employee was holding the leash for a large dog. The dog panicked and pulled on the leash, causing the employee to fall forward to the ground. She suffered a broken kneecap and was hospitalized.

A cat bit a veterinarian on the thumb while being examined. The wound became infected and the employee was hospitalized.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
