An employee was in a verbal altercation with a coworker. The site foreman intervened and the employee began walking away. As the employee was walking away, the coworker assaulted him from behind with a punch to the head. He lost consciousness and fell into hot asphalt that had been freshly poured nearby. He was hospitalized with burns to the left side of the face, the left shoulder, and left forearm.

An employee was loading a crane onto a transport vehicle (low boy). The employee jumped down from the exit area of the crane and rolled his right ankle fracturing it.

An employee was in a ditch assisting with setting a water pipe. A pipe attached to machinery with a chain struck the employee's leg when the equipment was moved. The employee suffered a fracture under the knee cap that required surgery.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.

An employee was training on a jockey truck. Upon stepping out of the truck, the employee was struck by a rubber tire gantry crane, resulting in amputations of the right leg above the knee and left leg below the knee.

The employee s hand then became caught between the pop-up roller and the belts on the drop table. The employee sustained an avulsion to their right hand excluding the fingers. The employee was hospitalized and required surgery.

On May 6, 2025, a temporary employee was loading a sheet of fiberglass between two rollers on a roofing material machine. The top roller was not rotating. The roller fell and pinched his left ring fingertip, causing a partial amputation of the finger that required surgery.

An employee was about to dump sand from a hopper, raised on the forks of a forklift. As the hopper's weight shifted forward and it began to dump, the employee's left middle finger was pinched between the forklift fork and the metal frame of the hopper. The fingertip was crushed, and the employee underwent a medical amputation at the first knuckle.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.
