An employee was checking the vacuum on a machine when he noticed that the water level was getting high in the slurry. He discovered that pellets were clumping together; when he tried to stir the water, he received an electrical shock caused by a wire that had shorted out. He suffered head and neck pain and was hospitalized.

An employee was unchoking a friction washer system. He climbed on the floater table, which is 50 inches from the ground, and placed the lid back on the friction washer. As he was climbing back down, he slipped and his left leg fell into the auger, resulting in the amputation of the left leg above the knee.
Employees were preparing to adjust the blade on the grinder machine. Product was not completely clear from the grinder which prevented the carriage blade from rotating. An employee climbed up onto the carriage and used his weight to force the carriage blade to move. When it did, the employee stepped off the carriage and his left big and second toes were amputated by the blade.
An employee was monitoring the production conveyor to remove defective products from production. While he was pulling a large piece of plastic off the conveyor, the piece of plastic fell into the equipment. The plastic started making a loud noise, so the employee attempted to remove it.
