Final narrative
An employee was removing leftover material from the bed of a flatbed truck. He tripped on dunnage and fell off the flatbed, landing on and injuring his right elbow. He was hospitalized.
An employee was placing 4x4 timbers under a steel plate that was being loaded onto a trailer for delivery. The plate was raised about 12 inches by four chains with hooks on the four corners of the plate. One side of the plate was flat and the other side was raised; the middle section was beveled upward. While the employee was placing the timber under the plate with his left hand, the plate lost tension and fell, crushing the tip of their left index finger. The employee's fingertip was amputated.
