An employee was securing a bell hook by wrapping a strap around the A-frame of a side boom when the operator moved the winch, causing the strap to fasten around the A-frame and the winch mounting bracket. The employee's left hand was then caught between mounting bracket and A-frame. The employee suffered a broken left hand, a ring finger compound exposure, and broken middle finger.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

The employee was touching the forklift's metal frame at the time and was shocked on the left palm. The employee suffered burns to both the left palm and the sole of the left foot.
