Final narrative
On August 13, 2018, at 11:30 AM, an employee was cutting wood to length using a header saw when the saw amputated his left pinky fingertip.
An employee was operating a cap saw when a block of wood became wedged between the saw blade and the conveyor. The employee turned the saw off and was reaching under the saw to remove it when the blade lacerated the employee's hand.
An employee was attaching oriented strand board (OSB) sheathing to exterior wall paneling using a staple bridge machine and was caught in the in-feed chain resulting in partial amputation of the distal part of the right middle finger.
An employee was feeding lumber into a header saw when the saw was activated via the foot switch, causing it to cycle and amputate his left ring finger at the nail.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.

This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.
