An employee was lowering a pipe stabilizing tool called a "slip" into a drill hole/floor hole. As the drill was being lowered into the slip and drill hole, the employee's right foot lost traction and moved underneath the slip. The drill then entered the slip, which caused the slip to move and crush his right foot. His right big toe was severed at the first knuckle.
