An employee was drying and finishing vehicles when a vehicle door closed on their left middle fingertip and amputated it.

An employee was climbing down from a tractor trailer bed when they lost their footing and grabbed onto the roll-up door to stop the fall. The roll-up door then fell onto the employee's hand, resulting in a hand fracture that required surgery.

An employee was attempting to remove a tarp from a flatbed trailer when the trailer handle struck the employee in the chest, resulting in two broken ribs and a punctured lung.

An employee was near a backhoe while the operator was getting a broom. The broom handle struck the lever that actuates the swing of the backhoe bucket. The bucket then struck the injured employee, pinning him to the outrigger. The employee suffered crush injuries to the chest.

The injured employee was being trained on loading trucks in the load out bay. The injured employee and his supervisor climbed the stairs that run parallel to the truck and trailer, walked to the upper platform used for observation and sample taking, and saw that the lid of the grain trailer was closed. The employees began working to open the lid which appeared to be stuck due to cold weather. The injured employee pulled on one of the pivot arms that connect the lid to the main body of the trailer. As the lid began to open, the employee s right hand slipped down on the pivot arm and his index finger was caught between the pivot arm and the base of the pivot arm. The employee s right index finger was fractured and three nerves were severed. The employee was hospitalized.

An employee was setting a rebuilt water pump using a forklift and a chain after inspecting the area where the pump was going to be placed. While the employee's right hand was on a nearby valve flange, the water pump shifted while in the air and struck the employee's right index finger, fracturing it. The employee was hospitalized and required surgery.

A waste water technician was retrieving a tool to adjust a gauge on a water pump when a copperhead snake bit the employee's right hand. The employee was hospitalized.

An employee was at the top of the ladder on the passenger side of a tanker trailer. While the tanker was being loaded, the employee was checking the level of the liquid inside the tanker by looking through the manhole on top of the tanker. The employee may have been overwhelmed by hydrogen sulfide (H2S) or the odor of raw sewage, causing him to lose consciousness and fall approximately 10 feet to the ground. The employee sustained a brain bleed and fractures to their skull, face, spine, and multiple ribs.

An employee was working in a wastewater facility, cleaning a metal screen. He positioned it against a railing on top of a metal grating, which was above a headworks bypass channel. When he stepped onto a trapezoid-shaped section of grating, it slid forward and tipped into the channel. The employee fell about 5 feet into the channel, which contained about 1 foot of stagnant wastewater. He suffered a rotator cuff and ligament tear in his left shoulder, a contusion on the left side of his back, a broken left-side rib, and a tear of the medial collateral ligament in his right knee.

An employee was lifting a compactor when it slid in his hand and caught his right little finger between the compactor and the truck trailer, resulting in a partial amputation.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
