An employee was retrieving a pallet from a customer and cutting off the shrink wrap. The employee reached to push the shrink wrap into a dumpster when the forks on the powered industrial vehicle tilted and crushed his fourth and fifth fingers. The employee sustained an amputation of the fourth and fifth fingers.

An employee was putting a mold lifting cylinder on a chain fall hoist. The cylinder shifted and pinched the employee's left index finger against its mounting bracket. The fingertip was amputated.

At 3:36 p.m. on June 11, 2021, an employee was using a cutting torch to cut the end off a 55-gallon drum that had been filled with hand sanitizer. The drum flashed and the employee suffered a thermal burn to the left side. He was hospitalized.

On 9/11/2017, at 5:20 p.m., an employee was cleaning out a mixer when he fell through a grate on the floor and struck the grate plate, injuring his groin area.

TForce Freight

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

Liveo Research, Inc.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
