At 3:36 p.m. on June 11, 2021, an employee was using a cutting torch to cut the end off a 55-gallon drum that had been filled with hand sanitizer. The drum flashed and the employee suffered a thermal burn to the left side. He was hospitalized.

The employee reached to push the shrink wrap into a dumpster when the forks on the powered industrial vehicle tilted and crushed his fourth and fifth fingers.
