An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

On December 27, 2023, an employee was delivering mail to an apartment complex when their knee gave out and they fell to the ground. The employee was hospitalized with a left tibia fracture.

An employee was walking to the inventory cage when her ankle gave out and she fell to the floor. The employee sustained a femur fracture and was hospitalized.

An employee was dislodging a pallet that was stuck when boxes of product fell onto them, causing them to fall to the ground. The employee suffered a fractured femur.

An employee was working at a quench conveyor belt, pulling steel blades off the belt and placing them on the low heat belt. One of the blades fell off the quench belt onto the floor. The employee bent over to pick the blade up off the floor when another blade fell off the quench belt and struck them on the top of their head. The employee was hospitalized with a brain bleed.

An employee was operating a 32-ton mechanical power press performing a presetting function when a finger on his left hand was caught in the press and the fingertip was amputated.

An employee was charging a gas spring to pressure when the gas spring blew apart, injuring his right hand, left hand, and abdomen. The employee suffered the amputation of three fingers on his right hand and was hospitalized.

An employee was conducting a measurement setup on a parabolic eye forming apparatus. During the setup process, the forming assembly cycled and the employee's left index finger was crushed between the forming assembly and the die block. The employee sustained a laceration and fracture to the left index finger that resulted in medical amputation of the fingertip.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
