An employee was operating a pattern block roller machine. The employee was pinned (at the mid- to upper torso) between the pusher bar and the assembly platform. The employee suffered broken ribs.

An employee was preparing to wash out a concrete mixer truck. When the water was turned on, the employee lost balance and fell into the corner of the top rail. The bolt securing the rail failed and the employee fell approximately 7.5 feet, injuring his left knee.

On July 27, 2021, an employee was on a straight ladder to check the level of an open aggregate hopper when the ladder slipped, causing him to fall approximately 3-4 feet and resulting in a broken left ankle.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

On September 30, 2025, an employee was applying production labels to a splintered piece of pallet on a conveyor. Their right middle finger was caught as the split line exit conveyor moved forward, resulting in an amputation.
