During a fiber placement job, an employee needed to remove a branch from an existing cable strand. He put his 24-foot extension ladder on the strand and cut the branch; the strand then broke and he fell about 15 feet. He suffered a broken elbow, broken ribs, and a bruised kidney.

An employee fell approximately 10 feet while attempting to descend from the top of an office (mezzanine) that is located inside an aircraft hangar. He was hospitalized with two broken wrists.

An employee was attempting to repair a cable from a ladder. As he descended, the ladder moved and he fell. He got caught between the steps and broke his right leg.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

On June 16, 2025, at 4:30 p.m., an employee was ascending an 8-foot A-frame ladder to look inside an attic opening of a customer's garage. The employee was three rungs from the top when he fell 5 feet to the ground, resulting in a displaced fracture of his femur in the back of his knee. The employee was hospitalized and required surgery.

An employee was about to install a cable at a residential site. While setting up a ladder, he stepped back on an uneven street surface and hyperextended his knee. He dislocated his knee and tore his patellar tendon.

An employee was descending a ladder when it shifted, slid, and twisted. The employee fell about 12 feet to the ground and broke his left hip. He was hospitalized and required surgery.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
