Final narrative
An employee was inserting the final bolt to install a 1,000-pound electrical substation switch on a metal frame. The switch was supported by a magnetic lifting device, which released the switch; it dropped about 2 inches and pinched the employee's left hand against the metal frame. The employee suffered injuries requiring surgery: two broken bones in the hand and lacerations to the index finger and thumb.
An employee was operating a core knock-out machine when a cylinder on the pneumatic shaker pinched his left middle finger, resulting in a fingertip amputation.
