An employee was manually stuffing a tote liner inside the torn opening of a sack full of peanuts that was positioned on a pallet on the ground when another sack stacked on top of the sack the employee was stuffing fell on and pinned him against the ground. He sustained a right ankle fracture and lumbar (L5) vertebrae fracture.

An employee's left hand was caught by the butterfly valve inside a bulk line sugar feeder. The machine amputated his index, middle, and ring fingertips at the first knuckle.
