Federal OSHA recorded a severe workplace injury at The TJX Companies, HomeGoods Distribution Center, BLOOMFIELD, CONNECTICUT 06002 on — Fractures, affecting the multiple foot (feet) locations.

Two forklifts were traveling in the same direction. The operator of the first forklift realized he had gone too far and stopped to turn around. When he started to turn, one of the forks struck the employee who was operating the second forklift in the left foot, causing a possible fracture to the foot/toe.

Apex Industrial Services

An employee was traveling down an aisleway in a three-wheeled golf cart. They were moving around a parked forklift when a material cart came from the opposite direction. A wheel on the material cart caught a piece of racking causing the material cart to swing out and hit the injured employee's golf cart. The material cart continued to swing, hitting the injured employee's left leg. The injured employee was also knocked out of the golf cart and landed on the ground 2-3 feet away. The injured employee sustained fractures to the left leg, requiring hospitalization and surgery.

Joe McGee Construction

An employee was riding on a flatbed trailer when his left foot was caught between the trailer and a trackhoe. He sustained a laceration to his left foot that required stitches and hospitalization.

Menzies Aviation

Employees were operating a baggage tractor on a service road. The tractor made contact with an aircraft's left wing and the seat of the tractor was ripped off. The employees were thrown to the ground. Two employees were hospitalized with injuries including a chipped tooth, bruises and abrasions, a dislocated hip, a fractured humerus bone, and a laceration to the left knee.

Expressjet Airlines

An employee was driving his vehicle (golf cart style) to an aircraft when he passed by two large baggage carts and was T-boned by another vehicle. He was hospitalized with a broken left leg.

An employee was operating a forklift, which had a T-bone collision with another forklift. The employee suffered four broken right ribs, a bruised right lung, and skin bruising and was hospitalized.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

A temporary employee was punched in the face by another temporary employee. He suffered a laceration to the eye.

An employee was backing up a powered industrial vehicle. Her left hand was caught between the vehicle and a rack, and the wrist was broken.

An employee was operating an extruder and performing a spool swap over from the right spindle to the left spindle. After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.

An employee was resetting a warehouse racking system utilizing an order picker. They fell approximately 5 feet from the order picker to the concrete floor and sustained fractured ribs.

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

An employee was working to clean a glue roller with a scraper tool. The tool and the employee's left thumb were pinched between two rollers, resulting in a partial amputation.

United Refrigeration Inc

An employee was re-arranging boxes of pipe insulation on a pallet. One of the boxes fell from the pallet and knocked over an upright empty cylinder. The cylinder fell and crushed the tip of the employee's right toe. The employee's toe required surgical amputation.

Michels Power, Inc.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

Parker-Hannifin Corporation

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.
