An employee was cutting logs with a billet saw. The saw blade broke, causing amputations to the employee's left little, ring, and middle fingers. The employee was hospitalized.

Employees were lifting a metal plate with a chain and hoist. The steel plate was to be modified/outfitted with eyehooks. Once the eye hooks were welded onto the plate, the plate was lowered. As the plate was being lowered one of the welds failed causing the plate to fall and land on the employee's foot resulting in an amputation.
