An employee was operating a drawing bench. The rollers caught the employee's shirt, then the glove on his right hand. The rollers then pulled in his hand, breaking four fingers.

An employee was using an overhead crane to load a bundle of hexagonal bars onto the conveyor belt of a die bench machine. When the employee unstrapped the bundle, a hexagonal bar moved and crushed the tip of his left thumb against another bar resulting in a fingertip amputation.

An employee was operating a continuous draw machine when his right hand entered the point of operation, resulting in the amputation of the two middle fingers.

An employee reached into the bench area of a draw bench to retrieve a piece of wood for cribbing for steel bundles. The operator of the draw bench dropped a steel bar which fell onto the employee's right hand, causing dislocated finger bones (requiring resetting), cuts (requiring stitches), and bruises.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.

An employee was removing a roll (tooling) from a machine when their left little finger was pinched between the roll and the machine, resulting in a partial amputation.

An employee was rolling a piece of steel in the belt of a shape roller machine when his finger was pinched between two pieces of steel. His right ring and middle fingers were amputated.

An employee was cutting a length of steel coil when he noticed the steel might have cross-break defects. He used his gloved hand to feel for and clean the defected spots when his glove got caught in the measuring wheel, which rolls along the top of the steel to ensure accurate cuts. The employee's arm was pulled down into the machine until his body was stopped by a support bar that goes across the machine. The employee's arm was fractured at the elbow and required hospitalization.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
