The employee removed an access cover to the auger and the auger grabbed his gloved left hand, amputating the tips of his left index and ring fingers and his little finger at the third knuckle. The mixing machine was in operation at the time.

When the employee went to gain support on a stadium light tower, he was shocked by an electrical panel.
