An employee was starting to collect edge trim when their right middle and ring fingers came into contact with a shear slitter blade. Half of the ring finger was nearly amputated, and half of the middle finger was amputated.

An employee was feeding film through stainless steel rollers on a lengthwise orienter machine when their left forearm became caught between two rollers. The employee sustained fractures to their radius, ulna, and wrist, as well as an avulsion to the left forearm.

An employee was tying a feed bag when his hand slipped and struck the side of the feed hopper resulting in a laceration to the left wrist. The employee was hospitalized.

This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a stand-up forklift to move a pallet of product that was sitting on the floor in an aisle of the finishing warehouse next to metal racking. The forklift cab went under the middle shelf and his torso became caught between the metal shelf (at his back) and the forklift steering control stick (at his front). He was hospitalized with possible fractured ribs.
