An employee was operating a 40" rubber mill and clearing a jam when his gloved left hand was caught and pulled in between the rollers of the mill. All four fingers on the left hand were amputated. The employee was hospitalized for surgery. The machine was guarded at the time.

An employee was working on a filament winding machine when his finger became stuck in the fiberglass. He sustained a right middle finger amputation at the first digit.

An employee was about to reposition a mandrel by hand when a puller engaged, causing the tooling to press against the puller plate. The employee's ring fingertip and nail were lacerated.

An employee was investigating smoke coming from a 55-gallon drum of resin sitting on a heated rack. After investigating the smoke, he began walking away from the drum; it ignited, producing a ball of fire. He suffered second- or third-degree burns to the right side of his face, back, arm, and knee area. He was hospitalized, requiring surgery.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was working with fiberglass and resins. The employee was hospitalized after having difficulty breathing caused due to chemical exposure.

The injured employee was letting air out of a winding shaft using scissors as a lift table was used to pull the roll off the shaft. The roll/cardboard core slid over the top of the injured employee's scissors and caused the scissors to amputate the employee's little fingertip at the nail base.

An employee was dropping off a skid of laminate to be cut at a table saw. The skid was lifted and the forks tilted forward when the stack slid forward and struck the injured employee. The employee was pinned between the laminate and the table of the saw, resulting in a back fracture and a laceration to the liver.

An employee was using a machine to cut a roll of plastic sheeting. The employee's right hand was at the end of the roll, and when the cutting mechanism of the machine cycled, the employee's right middle and ring fingers were cut, resulting in amputations to both fingers.

An employee was running acrylic film through an automated winding machine which wraps film around a cardboard cylinder core. He noticed that the film had slipped off the winder. He went to the front of the machine to re-attach the film to the winder spool. His fingers got caught between the film and the spool and his left arm was pulled up by the winder spool. The employee was hospitalized with a broken left forearm. Another employee injured their shoulder when pulling the first injured employee out of the machine.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
